Big spoilers here....
To be honest, if you go by The Phantom Pain's story, the guy that solid snake fights is not THE Big Boss. He's fighting Venom Snake... or Medic as some people in the MGS community like to call him. However, on the codec he's talking to the real big boss.
Soooo that old guy snake is talking to at the end of MGS4 is the real big boss as well.
